The Broad Street Cemetery Inventory Forms collection consists of information about each of the graves in the Broad Street Cemetery in Salem, Massachusetts. Each form is filled out with the deceased's name (if legible), death date, motif of marker, orientation, dimensions, description of plot, and conditions of plot and the conditions of the plot in general. On the back of many of the forms is the name, epitaph, etc., inscribed in the stone. The quality and quantity of information on each form varies greatly. In some cases, the processor was able to fill in names or letters on the forms using the printed vital records for Salem, Mass.; this information has been penciled in brackets on the forms.

The forms have been arranged alphabetically. See index I for a complete list of included names. Many of the names have more than one form for the graves, usually one for the headstone and one for the footstone. Occasionally, more than one person with the same name is buried in the cemetery; all forms for the surname in which you are interested can and should be examined. The "see" references in the Appendix refer to someone's name occurring somewhere on the marker; it may or may not reflect the fact that the person referred to is buried in the gravesite.
